About this role

Provides comprehensive primary care services across the lifespan, including preventive care, acute illness evaluation and treatment, chronic disease management, same-day access visits, and care coordination; collaborates with physicians and interdisciplinary team; conducts assessments, develops diagnostic/treatment plans, orders and interprets results, prescribes therapies within scope, coordinates care with specialists; participates in quality improvement, patient safety, population health, and operational initiatives; serves as clinical resource and provides education and mentorship to learners.
