About this role
Provides clinical pharmacy services to ambulatory patients, including comprehensive medication management across multiple areas (anticoagulation, perioperative anticoagulants/antiplatelets, anemia management, diabetes, hypertension, hyperlipidemia, heart failure, COPD, asthma, and opioid use disorder) and ensures departmental efficiency and regulatory compliance; instructs other pharmacists, residents, and students; the Medication Management Clinic provides care coordination for high-risk patients.
