About this role
Clinical Pharmacist-Outpatient I provides evidence-based pharmaceutical care including pharmacotherapy consults, disease management support, medication reconciliation, polypharmacy assessment, care plan support, DUR reviews, and dispensing verification within the retail pharmacy workflow; conducts drug utilization analysis, supports formulary management, and collaborates with healthcare teams to optimize patient outcomes using EPIC for documentation.
