About this role
The Pod Provider delivers comprehensive chronic disease management, evaluation of acute concerns, behavioral health management, and preventative care within primary care, collaborating with an interdisciplinary team to develop member care plans and support care journeys; responsibilities include conducting intake visits, providing longitudinal primary/ wrap-around care, proactive member follow-up, comprehensive documentation and coding, prescribing medications for depression/anxiety/substance use disorders, staying updated on evidence-based care for complex patients, referring to specialists as needed, and participating in care-team case conferences.
