About this role
In an inpatient setting, remains accountable for provision of direct care to assigned patients; 12 hour day shifts with E/N and 8 hour shifts; Hospital at Home program providing inpatient-level care at home; responsibilities include admissions, discharges, remote patient monitoring, care coordination and logistics management; minimum 2 years acute care/ICU/ED experience; NYS driver’s license required.
