About this role

The CNS or NP serves as a clinical resource and an independent practitioner who diagnoses and manages acute and chronic diseases in the area of advanced practice licensure. Proactively manages the care of patients across the continuum of care. Oversees the care management of complex patients. Daily Schedule: Monday-Friday 7:30am-4:00pm; No call obligation; 14-16 patients per day; Wound Treatment Assistants available per provider; Chronic wound & ostomy management: evaluation and assessment, diagnosis, treatment, including debridement of wounds and application of appropriate dressings, establish plan of care and maintain continuous treatment; Student Loan Forgiveness eligible under PSLF and NM Health Professional Loan Repayment Program; Masters degree in Nursing. Current New Mexico advanced practice nursing as CNS or NP license required. Three to five years experience in nursing with one to three years experience in area of expertise. Requires Wound Specialist certification within 1 year of hire. Case management and/or home care experience preferred. New Mexico Board of pharmacy licensure; DEA, and CNS or NP licensure required.
