About this role
As a practicing physician, provide medical consultation and/or support to other practitioners within the organization; diagnose and treat acute and chronic medical conditions; perform comprehensive health assessments; develop and implement proactive patient-centered care plans; provide compassionate palliative and goal-based care; prescribe medications and monitor treatment; establish relationships with caregivers and care facilities; educate patients and families on preventive and chronic care needs; assess and counsel patients on preventive needs and services; licensed MD/DO in Oregon with internal medicine or family medicine residency, ABMS certification, DEA license, valid driver's license, CPR certification, geriatric/palliative care certification preferred, and experience in medical home visits; knowledge of urgent care/triage for relief care; proficient in geriatrics and palliative care; hybrid work location with 2 days/week on-site; location requirement for Marion/Polk counties within 15 miles of county assignment.
