Active NP/PA license with 3+ years of outpatient primary care experience; telehealth-focused role conducting comprehensive assessments, chronic condition management, preventive care, and care coordination in a remote, tech-enabled environment; involves telehealth video visits and occasional in-home visits in local area; requires BLS; willing to obtain additional licenses if necessary within 90 days; familiarity with managed care (STARS/HEDIS) and Medicare preventive visits; member-centered, preventative primary care in a mission-driven team.
