About this role
The IHC Nurse Practitioner is a home-based, hybrid in-home and remote role within Devoted Medical's Intensive Home Care program, collaborating with an interdisciplinary team to provide whole-person clinical, social, and logistical support for patients with complex chronic conditions and high risk of hospitalizations.
