Routine Primary Care APP (NP/PA)- Telehealth

About this role

Provide virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions; diagnose and manage chronic conditions, develop individualized care plans, and treat acute concerns within virtual care; order/interpret lab tests and imaging; manage medications and refills; provide health coaching and patient education; coordinate referrals and home-based services; contribute to electronic health platform improvements; participate in clinical case reviews and team huddles; requires active APRN or PA license and ability to obtain licensure in multiple states within 90 days; minimum 2 years of primary care/urgent/internal medicine experience, preferably telehealth; comfortable with digital documentation and EHRs.
