About this role

As an RN Case Manager, you will provide skilled nursing care in the home while guiding each patient’s plan of care from start to finish. Responsibilities include assessing, planning, implementing, and evaluating individualized patient care; developing care plans with physicians and the interdisciplinary team; completing admissions and ongoing nursing assessments; educating and supporting patients and families; and documenting care accurately and in a timely manner.
