About this role
Deliver comprehensive hospice services in accordance with the Nurse Practice Act under a collaborating physician's supervision; conduct patient evaluations, health assessments, diagnosis and management of care plans, including prescribing medications and treatments; perform physical exams and hospice eligibility evaluations in collaboration with the interdisciplinary team; coordinate care with patients, families, and healthcare practitioners; utilize community resources to develop and direct services at home and in the community.
